Download, print, and fill out the “Paper Background Check Form.”
Click on “Online Background check form” and use code 2F1J3Y to schedule a fingerprinting appointment. Contributor Case Number = D16004
Bring your completed “Paper Background Check Form” and your ID or passport to the appointment.
Note: You MUST do prints in New Jersey. Pennsylvania will not be accepted. Please pay the day of, not ahead of time
